North Woods Community Health Center Project Director for Patient Centered medical home Part Time

Job Description

North Woods Community Health Center seeks a Project Director to help organization achieve the standards of patient centered medical home (PCMH). Immediate start, about 20 hours/week for first year. Must have excellent analytical skills, strong writing skills, be well-organized, display initiative, and enjoy assessment against NCQA accreditation standards.

In addition to a clinical background, ideal qualifications include Bachelor's degree; familiarity with PCMH purpose and goals; 5 years relevant ambulatory care experience, work with electronic health records and team collaboration; strong track record of successful project management.

North Woods is a growing primary care health center delivering medical and dental care with a charitable mission to reach the uninsured and underinsured. Clinics located in Hayward and Minong, WI. Opportunity for this position to expand over time to other leadership projects or become full-time.
